DESCRIPTION:Are you at risk for type 2 diabetes? Ready to make a change? If you have prediabetes (your sugar is high) or believe you are at risk for diabetes you may be eligible for this free online program. Part of the National Diabetes Prevention Program\, we work with a CDC curriculum and a trained Lifestyle Coach. \nLifestyle coaches help participants reach the following goals: \n\nLose a minimum of 7% of starting weight\nIncrease physical activity to 150 minutes or more each week\n\nTo register or for more information visit www.putnamils.org/diabetes-prevention-program then fill out the eligibility questionnaire and registration. \nJoin us for the first session on January 14 and start a full year of support to help you prevent type 2.
