We’ll guide you through how to get into the right mindset, gather essential documentation, and engage your teen in conversations about their strengths, needs, and goals for the future. You’ll also learn how to: Develop a transition-focused plan that addresses career readiness, independent living skills, and post-secondary education or training options. Understand how to include your teen’s voice in setting meaningful goals for life after high school. Identify when to bring additional support, such as advocates or service providers, to the meeting. Follow up effectively after the meeting to ensure plans are put into action and monitored over time.
