Ready, Set, Work!

A work readiness program for individuals who have been involved in the criminal justice system using cognitive and scientific assessment

Ready Set Work is an intensive program that helps those under probation/parole supervision with proper application creation, interview skills, overcoming barriers, job search strategies, retention planning, and skill developments.

Regional Resources Development Assistant (RRDA)

In addition, WILC/PILS operates a Regional Resources Development Services Program (seven counties). The goal of this Program is to develop home and community-based services and supports needed by individuals and family members who experience a disability.

POSITION:                              Regional Resources Development Assistant (RRDA)

HOURS:                                   Full time, 35 hours per week, with benefits

RESPONSIBILITIES:

  1. Review, track and monitor Serious Reportable Incidents and Recordable Incidents for the LHV
  2. Tracking documentation in the DOH and Agency database i.e. service plans, team meetings, trainings, outreach, phone contacts;
  3. Communicate with applicants and providers when documentation is late or needed
  4. Maintain files on all waiver participants;
  5. Provides information and referral to services available to individuals with disabilities in the lower Hudson Valley;
  6. Assist with preparation and submission of all reports/surveys and tracking forms;
  7. Assist in arranging and providing confirmation of completion for required trainings/workshops for Hudson Valley TBI providers and community;
  8. Assist/Conduct eligibility screenings to individuals seeking wavier services in the region as needed
  9. Attend required meetings and conferences for DOH waiver and;
  10. Conduct any other duty as assigned by Lead RRDS/SR RRDS that will promote and enhance the development of the waiver in the Hudson Valley Region;

QUALIFICATIONS:

  • Bachelors Degree or equivalent experience
  • Demonstrated abilities in written and oral communication skills
  • Ability to use word processing computer programs

Administrative Assistant

10.15.2019 WILC is a consumer controlled, nonresidential center for independent living, which provides information and advocacy to persons with disabilities.  In addition, WILC operates a Waiver Services Program.  The goal of this Program is to develop home and community-based services and supports needed by individuals and family members who experience a disability so that they can live in the community.

POSITION:                       Administrative Assistant

HOURS:                              F/T 35 hours per week

RESPONSIBILITIES:

  1. Conduct all phone screenings for the Waiver programs.
  2. Assist with tracking Waiver Housing Documents and following up on phone calls.
  3. Document all phone complaints and fill out complaint form
  4. Schedule/create/distribute information/forms regarding all provider meetings.
  5. Attend regional provider meetings as needed, and track provider attendees
  6. Track all new phone inquiries into agency’s database
  7. Assist Track all DOH waiver documentation such as, service plans, PRI/Screens/UAS, participant enrollment to ensure that all approved providers are within the given DOH timelines outlined in the manual.
  8. Mail out all initial enrollment packets to waiver applicant
  9. Establish and file all DOH waiver documentation as requested by waiver staff
  10. Disburse any new DOH waiver information/survey to all approved providers via e-mail, fax or mail.
  11. Other duties and projects as assigned by Supervisor/ Dep Executive Director.

QUALIFICATIONS:

  1. Associates Degree or High School diploma w/ two years professional experience.
  2. Excellent communication, interpersonal and organizational skills.
  3. Ability to multi-task and handle multiple request from fellow staff.
  4. Dependability, approachability and willingness to take initiative and problem solve.

Two opportunities for affordable/accessible apartments

10.1.2019 Affordable/Accessible Rental Units

Hillcrest Commons Phase 2, Carmel

WILC/PILS is in a contractual agreement with Hillcrest Commons Phase 2 in Carmel, an affordable housing development for seniors and frail disabled (their description) 55+.  We have 12 set aside units and are given priority for referrals for these units.

The deadline for applications is October 9, but since we have priority status, this deadline can be extended for our referrals.

The Hamlet at Pawling

WILC/PILS has 12 set aside units at the Hamlet at Pawling and posts vacancies as they occur.

The Hamlet at Pawling has a fully accessible two bedroom apartment available.  The rent is $1043 with a one month security deposit.  Applicants must be 55+ years of age with a maximum income of $41, 640.

Transition Specialist – Open Doors Transition Center

Westchester Independent Living Center, Inc. (WILC) and its satellite office Putnam Independent Living Services (PILS) is a consumer directed, non-residential, information, referral and advocacy center serving people with disabilities, seniors and veterans. We currently have an opening for a Full-time Transition Specialist in our Open Doors Transition Center Program based out of our Putnam County office in Brewster, NY. The Transition Specialist assists individuals to transition out of institutional settings by educating them on community- based services and assisting in the development of a discharge plan that will facilitate their move back to the community. They also provide follow along assistance for 1 year period post discharge.

POSITION: Transition Specialist, Full-time
REPORTS TO: Program Coordinator

RESPONSIBILITIES:

  1. Coordinates visits to nursing homes and other institutions to provide residents, families and discharge planners with objective information about available home and community based and other potentially necessary services which may assist them to transition back to the community.
    2.    Participates in discharge planning meetings and coordinates with other relevant social service and community-based agencies involved in the discharge and transition care of the resident.
    3.   Explains the peer mentor program to residents and refers those interested in the program to it.
    4.   Ensures all significant consumer data and service provision details are currently and accurately documented into the WILC/PILS and NYAIL databases according to each agencies policy.
    5.   Develops, facilitates and maintains the Community Education Checklist which includes a Conflict Free Person-Centered Plan for transition.
    6.   Participates in various long-term care coalitions, human service councils and outreach efforts as requested by Program Coordinator.
    7.    Attends agency sponsored in-service trainings, activities; program and staff meetings.
    8.    Performs additional duties as requested by Program Coordinator and/or Director.

QUALIFICATIONS:
Bachelor’s degree in a human service field and two years of relevant experience working in a human service environment and/or with community based long term care services programs. An appropriate combination of skills, experience and education can be accepted in lieu of a Bachelor’s Degree. Experience with case management, service coordination or care management is desired. Candidate should be familiar with various community based services, HCBS Waiver and MLTC programs; be comfortable working with people with physical, mental and intellectual disabilities.  The candidate will have excellent written, oral communication and organizational skills; be computer and internet literate, able to work independently and as part of a team. They should have a commitment to self-direction, person-centered planning and consumer choice. Reliable transportation on a daily basis is required as program covers a 7 county region including the counties of Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster and Westchester. This position will focus on facilities located in the Orange, Sullivan, and Dutchess counties. Some evening and weekend availability may be required. Bi-lingual (English-Spanish is a plus).

Housing Specialist – Regional Lead

WILC/PILS operates a Regional Resources Development Services Program (seven counties). The goal of this Program is to develop home and community-based services and supports needed by individuals and family members who experience a disability.

POSITION:                                Housing Specialist: Regional Lead

HOURS:                                   Full time, 35 hours per week, with benefits

RESPONSIBILITIES:

  1. Westchester Independent Living Center (WILC), Housing Specialist will act as the regional lead and provide housing specialist for the OHS program, serving the counties of Dutchess, Orange, Putnam,  Rockland, Sullivan, Ulster and Westchester and working with Westchester Disabled on the Move, Resource Center for Accessible Living, Taconic Resources for Independence and Rockland Independent Living Center as auxiliary ILC(s).
  2. Regional Lead Housing Specialist (RLHS) will work collaboratively with Open Doors (MFP) program staff to identify individuals in nursing homes that are eligible for the Olmstead Housing Subsidy (OHS). RLHS will help identify strategies and goals for local outreach, and identify partnerships to be developed, both statewide and locally.
  3. Westchester Independent Living Center will hire the full complement of staff as outlined in the attached budget. Any proposed changes to the budget, including the staffing complement, must be approved by NYAIL prior to implementation. As part of the hiring process, ILC will identify a lead staff who will interface with NYAIL throughout the project.
  4. RLHS will accept referrals from nursing facilities, transition specialists, local administrators, consumers, family members, landlords, advocates and from NYAIL. RLHS will assign a housing specialist from their region (include ILC and auxiliary ILCs) based upon location of participant and availability. RLHS will monitor caseloads to ensure maximum productivity of housing specialists.
  5. RLHS will determine OHS eligibility and if appropriate, will search for housing that will meet the needs of the individual including affordability and accessibility. RLHS will follow OHS procedures for inspection of property and approval of subsidy.
  6. The assigned housing specialist will work with participants to identify needed community transition services (CTS) such as security deposits, utility deposits and essential household furnishing. ILC will follow OHS procedure for approval of CTS. RLHS will provide CTS and will bill fiscal intermediary as per OHS procedures.
  7. RLHS will utilize the database, created by NYAIL, to provide information about contacts, participant needs and services provided. ILC will have technology available to OHS staff sufficient to complete web-based reporting in database.
  8. RLHS will use outreach materials provided by NYAIL.
  9. RLHS staff funded through this MOU will avail themselves of training developed by NYAIL. ILC will identify training needs of its OHS staff, so that NYAIL can incorporate the information in scheduled training.
  10. RLHS will complete all reporting requirements for OHS, as developed by NYAIL. This includes but is not limited to timely submission of invoices, request for tenancy approvals and community transition services requests.
  11. RLHS will document and share with NYAIL barriers encountered that hinder stable housing. ILC will gather information from auxiliary ILCs about barriers encountered. With NYAIL and other OHS providers, RLHS will suggest solutions to NYAIL that will be provided to the New York State Department of Health to address barriers.
  12. ILC/RLHS will ensure all staff entering areas where patients/clients/residents/consumers might be present, including institutions, clinics, and homes, will either be vaccinated for the current influenza season or will have on file a signed agreement that the staff will wear a facemask. ILC will maintain a record of proof of vaccination for each staff person and provide a copy of this documentation to NYAIL.
  13. RLHS will provide technical assistance to auxiliary ILC(s) in their region as needed.

Thomas P. DiNapoli, NYS Comptroller asks major corporations to demonstrate disability inclusion

1.22.2019 As a longstanding supporter of equality, diversity and inclusiveness in the workplace, I recently wrote to a number of major corporations that our state pension fund invests in regarding disability inclusion.

Companies that embrace an inclusive workplace have access to a bigger pool of talent. In addition, a recent study “Getting to Equal: The Disability Inclusion Advantage,” found that companies with best practices for employing and supporting people with disabilities outperformed their peers.

Unfortunately, many companies have not taken adequate steps to demonstrate disability inclusion. I’ve asked some of the largest — Apple, McDonald’s, Twentieth Century Fox and others — to participate in the Disability Equality Index. This objective survey, an initiative of the American Association of People with Disabilities (AADP) and Disability:IN, helps companies self-report their disability policies and practices and identify areas where they can improve their reputations as employers of choice.

Protecting our state’s pension fund is one of my greatest responsibilities as State Comptroller. That’s why I call on companies to take advantage of opportunities to make improvements, and as a result, enhance the value of the fund’s investments.

Congress Passes Short Term Extensions: Medicaid HCBS Spousal Impoverishment Protections and Money Follows the Person

1.22.2019 On Thursday, the Senate passed the Medicaid Extenders Act of 2019 (H.R. 259). Now the bill has passed both the House and Senate, and is awaiting the President’s signature. It provides three-month extensions of both Medicaid home and community based services (HCBS) spousal impoverishment protections and the Money Follows the Person Program. Although these extensions are short they provide an important bridge to keep the programs going while we work with Congress on longer term solutions. November – National Diabetes Prevention Month

11.8.2018 – This November, Putnam Independent Living Services Recognizes National Diabetes Awareness Month and the Role of Inclusion in Prevention

This month-long campaign highlights a critical need for disability inclusion in diabetes prevention.

According to the Centers for Disease Control and Prevention (CDC) the rate of diabetes cases in the United States has nearly tripled over the past twenty years, particularly as the population ages and more adults become overweight or obese. What is often not highlighted is the number of adults at the highest risk for having Type 2 Diabetes, a condition also known as pre-diabetes. Approximately 84 million Americans or 1 in3 have pre-diabetes and most are not aware they have it.

In the United States people with disability make up approximately 25% of the total population and according to CDC, adults with disabilities experience a rate of diabetes more than double that of adults without disability (16.3% vs. 7.2%). Adults with disability also experience higher rates of obesity, hypertension and physical inactivity.

To raise awareness about diabetes, disability and healthy living, Putnam Independent Living Services is proudly participating in National Diabetes Awareness Month.

Joe Bravo, Executive Director of Putnam Independent Living Services (PILS), is enthusiastic about offering the CDC’s Prevent Type 2 program and said “Individuals with a diagnosis, or who have risk factors, are eligible for the program. Risk factors include being over 45, being overweight, having siblings or parents with type 2, also, for women, having giving birth to a baby weighing over 9 pounds or having had gestational diabetes.” He continued “Our healthy living specialist, a trained Prevent T2 Lifestyle Coach, offers workshops throughout the county, gives presentations and will work with employers and social service providers to bring the program to their employees or consumers.”

Putnam Independent Living Services (PILS) is the Putnam County arm of Westchester Independent Living Center (WILC) a nonprofit, community-based; consumer directed organization dedicated to improving the lives of people with disabilities. PILS has been providing advocacy and outreach in Putnam for over 10 years. WILC has a 30 year history of providing information, services, advocacy, technical assistance, and trainings in the Lower Hudson Valley Region to diverse and/or underserved populations. Our mission is to assist individuals with disabilities become as independent as they can be and to work within the community to provide education, disability awareness, and advocacy for the removal of barriers. The programs and services offered are designed by, directed by, and, in most cases, delivered by individuals with a wide range of disabilities. Although the bulk of the services are based in Westchester and Putnam, we also serve Dutchess, Orange, Sullivan, Rockland and Ulster Counties. Core services include benefits advisement, information and referral, and community outreach.
